WHAT IS ADULT CARE HOME CASE MANAGEMENT?

THE ASSESSMENT AND VERIFICATION OF THE NEED FOR ENHANCED CARE TO A MEDICAID RECIPIENT IN AN ADULT CARE HOME; MONITORING THE COMPLIANCE OF THE HOME IN PROVIDING THE PERSONAL CARE AS DETAILED IN THE PLAN OF CARE; AUTHORIZATION OF REIMBURSEMENT THROUGH MEDICAID AND RE-EVALUATION OF THE NEEDS OF THE RESIDENT WHEN SIGNIFICANT CHANGES OCCUR.

WHAT IS ADULT CARE HOME LICENSURE AND MONITORING?

THE RECRUITMENT, STUDY AND DEVELOPMENT OF ADULT CARE HOMES; EVALUATION, CONSULTATION AND TECHNICAL ASSISTANCE TO ASSURE COMPLIANCE WITH RULES AND REGULATIONS OF LICENSE; INVESTIGATION OF LICENSURE COMPLAINTS AND MONITORING OF ANY CORRECTIVE ACTION PLAN WHEN VIOLATIONS ARE SUBSTANTIATED.

WHAT IS ASSISTED LIVING?

GROUP HOUSING AND SERVICES PROGRAM FOR TWO OR MORE UNRELATED ADULTS THAT MAKES AVAILABLE AT A MINIMUM, ONE MEAL AND HOUSE-KEEPING SERVICES AND PROVIDES PERSONAL CARE SERVICES DIRECTLY OR THROUGH A FORMAL WRITTEN AGREEMENT WITH ONE OR MORE HOME CARE AGENCIES.

WHAT IS AN ADULT CARE HOME?

A RESIDENTIAL FACILITY FOR AN OLDER OR DISABLED ADULT WHO IS NOT ABLE TO LIVE INDEPENDENTLY, BUT WHO DOES NOT NEED NURSING HOME CARE. THE PURPOSE OF AN ADULT CARE HOME IS TO PROVIDE ASSISTANCE WITH ACTIVITIES OF DAILY LIVING.

WHAT IS THE DIFFERENCE BETWEEN AN ADULT CARE HOME AND A NURSING HOME?

NURSING HOMES PROVIDE CARE TO PERSONS WHO ARE CHRONICALLY ILL OR RECUPERATING FROM AN ILLNESS OR INJURY AND NEED REGULAR NURSING CARE AND OTHER MEDICAL SERVICES BUT NOT HOSPITALIZATION.

HOW DO I KNOW IF I NEED AN ADULT CARE HOME OR A NURSING HOME?

YOUR PHYSICIAN WILL DETERMINE YOUR LEVEL OF CARE BASED ON YOUR PERSONAL CARE AND MEDICAL NEEDS AND COMPLETE THE FL-2.

WHAT IS AN FL-2?

A MEDICAL FORM COMPLETED BY YOUR PHYSICIAN DESCRIBING YOUR DIAGNOSES, MEDICATIONS AND CARE NEEDED. THIS FORM IS REQUIRED PRIOR TO ADMISSION.

WHO LIVES IN THESE FACILITIES?

RESIDENTS CAN BE ELDERLY ADULTS, DEVELOPMENTALLY DISABLED ADULTS, MENTALLY ILL ADULTS OR ANY COMBINATION OF THESE.

HOW DO I CONTACT THESE ADULT CARE HOMES?

CALL THE DEPARTMENT OF SOCIAL SERVICES TO OBTAIN A CURRENT LIST OF HOMES IN YOUR COUNTY.

WHAT DOES IT COST? HOW DOES ONE PAY FOR THIS CARE?

RATES WILL VARY BY FACILITY DEPENDING UPON ACCOMODATIONS, SERVICES PROVIDED, RESIDENT’S INCOME AND ELIGIBILITY FOR ASSISTANCE. SEE EACH FACILITY FOR RATES. THOSE UNABLE TO PAY PRIVATELY MAY APPLY FOR SPECIAL ASSISTANCE AT THE LOCAL DEPARTMENT OF SOCIAL SERVICES.

ARE THESE FACILITIES LICENSED AND BY WHOM?

THE LICENSE IS ISSUED BY THE STATE OF NORTH CAROLINA THROUGH THE NORTH CAROLINA DIVISION OF FACILITY SERVICES AND MONITORED BY THE COUNTY DEPARTMENT OF SOCIAL SERVICES FOR COMPLIANCE WITH LICENSING STANDARDS UNDER THE SUPERVISION OF THE NORTH CAROLINA DIVISION OF FACILITY SERVICES.

DOES MEDICARE PAY FOR ADULT CARE HOME CARE?

MEDICARE DOES NOT COVER RESIDENTIAL CARE IN AN ADULT CARE HOME OR ASSISTED LIVING FACILITY.

WHO SHOULD I CALL REGARDING A COMPLAINT ABOUT AN ADULT CARE HOME?

CALL THE DEPARTMENT OF SOCIAL SERVICES IN THE COUNTY IN WHICH THE HOME IS LOCATED.

IS THERE A RATING SYSTEM FOR ADULT CARE HOMES?

NONE AT THIS TIME.

DOES SOMEONE SUPERVISE MEDICATION ADMINISTRATION?

YES, A MEDICATION TECHNICIAN WHO HAS BEEN CERTIFIED TO ADMINISTER MEDICATION.

ARE RESIDENTS FREE TO COME AND GO FROM THE FACILITY?

YES, UNLESS THEIR PHYSICIAN HAS DETERMINED THAT THEY WOULD BE AT RISK AND NEED 24 HOUR SUPERVISION.

MAY I TAKE MY PET?

MOST FACILITIES WILL NOT ALLOW PETS; HOWEVER SOME FACILITIES FEEL THAT HAVING PETS IS BENEFICAL TO THE RESIDENTS.

IS TRANSPORTATION FURNISHED BY THE FACILITY?

THE ADMINISTRATOR MUST PROVIDE TRANSPORATION TO NECESSARY RESOURCES AND ACTIVITIES, INCLUDING TRANSPORTATION TO THE NEAREST APPROPRIATE HEALTH FACILITIES, SOCIAL SERVICES AGENCIES, SHOPPING, RECREATIONAL FACILITIES, AND RELIGIOUS ACTIVITIES OF THE RESIDENT’S CHOICE. THE ADMINISTRATOR IS ALSO RESPONSIBLE FOR PROVIDING OR ARRANGING TRANSPORTATION FOR ANY MENTAL HEALTH DAY TREATMENT PROGRAMS IF THE PHYSICIAN MAKES IT A PART OF THE RESIDENT’S TREATMENT PLAN.

WHAT IS AT-RISK CASE MANAGEMENT?

A GROUP OF INTERRELATED SERVICES TO ADULT MEDICAID RECIPIENTS WHO ARE ASSESSED AS BEING AT-RISK OF ABUSE, NEGLECT OR EXPLOITATION AND IN DANGER OF PLACEMENT IN AN ADULT CARE HOME OR NURSING FACILITY.

WHAT IS HEALTH SUPPORT SERVICES?

ASSISTING INDIVIDUALS AND FAMILIES TO RECOGNIZE AND COPE WITH INCAPACITY AND LIMITED FUNCTIONING RESULTING FROM AGING, DISABLILITY AND HANDICAP(S).

WHAT IS MEALS ON WHEELS AND WHO IS ELIGIBLE?

PREPARATION AND DELIVERY OF NUTRITIOUS MEALS TO ELDERLY AND/OR
DISABLED ADULTS IN THEIR HOMES MONDAY THROUGH FRIDAY TO PREVENT
SOCIAL WORK PROGRAMS DIVISION

INSTITUTIONALIZATION OR MALNUTRITION. ELIGIBILITY BASED ON AGE AND/OR DISABILITY AND CURRENT NEEDS WITH CONSUMER CONTRIBUTIONS ENCOURAGED.

WHAT IS SERVICES FOR THE BLIND?

THE PROVISION OF SERVICES TO AID IN THE ADJUSTMENT TO VISION LOSS INCLUDING INDIVIDUAL AND FAMILY COUNSELING, INFORMATION AND REFERRAL, SUPPORT GROUPS, INSTRUCTION IN HOME MANAGEMENT AND PERSOANL CARE AND SAFE TRAVEL SKILLS.


WHAT IS REPRESENTATIVE PAYEE SERVICES?

THE DEPARTMENT OF SOCIAL SERVICES (DSS) SERVES AS REPRESENTATIVE PAYEE FOR THE SOCIAL SECURITY AND SSI BENEFITS OF PERSONS WHO ARE UNABLE TO MANAGE THEIR RESOURCES AND HAVE NO ONE ELSE WILLING OR CAPABLE TO ASSIST THEM. SOCIAL SECURITY AND THE INDIVIDUAL’S PHYSICIAN DETERMINE IF THAT INDIVIDUAL IS NOT ABLE TO MANAGE HIS/HER FINANCES. IF DSS IS NAMED AS PAYEE FOR AN INDIVIDUAL, A SOCIAL WORKER IS ASSIGNED TO THE INDIVIDUAL AS CASE MANAGER TO PROVIDE THESE SERVICES.

WHAT IS GUARDIANSHIP?

THE ADULT GUARDIANSHIP PROGRAM WORKS TO PROVIDE THE BEST POSSIBLE QUALITY OF LIFE FOR THOSE WHO HAVE A PUBLIC GUARDIAN APPOINTED BY THE LAW. OUR CLIENTS INCLUDE FOLKS WHO MAY SUFFER FROM ONE OR MORE ONSET ILLNESSES ASSOCIATED WITH ADVANCE AGE OR ADULTS WHO HAVE SUFFERED DEBILITATING INJURIES. GURADIANS SERVE AS SURROGATE DECISION MAKERS ACTING ON BEHALF OF THE BEST INTERESTS OF THE CLIENT. THESE DECISIONS TYPICALLY INCLUDE THE CLIENT’S NEEDS AND LIVING CONDITIONS. GUARDIANS ARE AVAILABLE AROUND THE CLOCK FOR EMEREGENCIES AND ARE PREPARED TO ADVOCATE PROPER CARE AND SERVICES.

WHAT IS PLACEMENT ASSISTANCE?

THE PROVISION OF COUNSELING AND OTHER SERVICES TO ASSIST INDIVIDUALS TO OBTAIN PLACEMENT AND ADJUSTMENT TO THE FOSTER CARE SETTING IN AN ADULT CARE HOME OR NURSING FACILITY.

WHAT IS WORK FIRST?

WORK FIRST IS AN EMPLOYMENT PROGRAM THAT IS DESIGNED TO PROVIDE TEMPORARY FINANCIAL ASSISTANCE TO FAMILIES THAT ARE PREPARING TO RE-ENTER THE WORK FORCE. BENEFITS ARE CONTINGENT UPON COMPLETING SPECIFIED HOURS OF WORK OR WORK RELATED ACTIVITIES PER WEEK. ELIGIBILITY IS BASED UPON INCOME, RESOURCES AND TIME LIMITS. YOU MAY NOT RECEIVE WORK FIRST FAMILY ASSISTANCE FOR MORE THAN 60 MONTHS IN A LIFETIME.

WHAT IS BENEFIT DIVERSION?

BENEFIT DIVERSION IS A LUMP SUM CHECK EQUIVALENT TO OR LESS THAN 3 MONTHS WORK FIRST FAMILY ASSISTANCE. YOU CAN NOT APPLY FOR BENEFIT DIVERSION. YOU MUST APPLY FOR WORK FIRST FAMILY ASSISTANCE AND BE ASSESSED BY A SOCIAL WORKER. THE SOCIAL WORKER DETERMINES WHETHER BENEFIT DIVERSION WILL BE OFFERED. ONCE YOU HAVE RECEIVED BENEFIT DIVERSION, YOU MUST WAIT 3 MONTHS BEFORE APPLYING FOR WORK FIRST FAMILY ASSISTANCE AGAIN.

WHAT IS JOB RETENTION SERVICES?

JOB RETENTION SERVICES ARE NON CASH SERVICES SUCH AS CAR REPAIR, UNIFORMS AND SHORT TERM TRAINING THAT HELPS FAMILIES MAINTAIN EMPLOYMENT.

WHO IS ELIGIBLE FOR JOB RETENTION SERVICES?

FAMILIES WITH CHILDREN UNDER AGE 19 AND STILL IN SCHOOL THAT MEET THE CITIZENSHIP AND KINSHIP RULES, HAVE INCOME THAT DOES NOT EXCEED 150% ABOVE THE POVERTY RATE, THAT ARE WORKING OR WILL BE STARTING A VERIFIABLE JOB CAN APPLY FOR JOB RETENTION SERVICES. APPLICATIONS OR INQUIRIES CAN BE MADE AT THE HAYWOOD COUNTY DEPARTMENT OF SOCIAL SERVICES DURING NORMAL BUSINESS HOURS.

WHAT IS THE FAMILY UNIFICATION PROGRAM?

THE FAMILY UNIFICATION PROGRAM IS NOT A HOUSING PROGRAM. IF A FAMILY IS FACING A SEPARATION DUE TO A VERIFIABLE HOUSING ISSUE, AN INCOME BASED HOUSING SLOT MAY BE RECOMMENDED. THE FAMILY MUST MEET MOUNTAIN PROJECTS ELIGIBILITY REQUIREMENTS AND MUST LIVE IN HAYWOOD COUNTY FOR THE LAST 6 MONTHS. APPLICATIONS CAN BE MADE AT THE HAYWOOD COUNTY DEPARTMENT OF SOCIAL SERVICES DURING REGULAR BUSINESS HOURS.
