PRIMARY PURPOSE OF ORGANIZATIONAL UNIT: The Community Alternatives Program for Disabled Adults (CAP/DA) is a specialMedicaid program under the Community Care Section of the Division of Medical Assistance.  The program makes care at home a realpossibility for many people who face nursing home placement.  When the program began in 1982, four counties participated.  It is nowavailable in all 100 counties.  This program will be under the umbrella of Social Work Programs in the department.

PRIMARY PURPOSE OF THE POSITION:  The Social Worker in this position is responsible for coordinating services through the Community Alternatives Program (CAP) to client, their families and caregivers.  This Social Worker will be responsible for a determined case load and be responsible for conducting home assessments, developing and managing a plan of care for each clientand to also monitor each client on a regular basis.  The Social Worker will complete all required documentation and detailed correspondence.

KNOWLEDGES, SKILLS and ABILITIES:  Considerable knowledge of social work principles, techniques and practices and theirapplication to individual casework group work and community problems.  Knowledge of the psychosocial, socioeconomic and behavioral problems and their treatment. Knowledge of governmental and private organizations and community resources.  Knowledge of laws, regulations and policies which govern the program.  General knowledge of medical terminology, diseaseprocesses and their treatment is required in certain programs or settings.  Skill in establishing rapport with a client and applyingtechniques of assessing psychosocial, behavioral and psychological aspects of client’s problem.  Ability to establish and maintain effective working relationships with administrative superiors, members of case load and their families, and with civics medical,social and religious organizations.  Ability to train or instruct lower-level social workers, students or interns.  Ability to express ideasclearly and concisely and to plan and execute work. 

MINIMUM TRAINING AND EXPERIENCE REQUIREMENTS: Bachelor’s degree in Social Work from an accredited school of SocialWork; OR Bachelor’s degree in Human Service field from an accredited college or university and one year directly related experience;OR Bachelor’s degree from an accredited college or university and two years directly related experience.  

TRAINING/EXPERIENCE:  Ongoing education and training are strongly encouraged within the Agency.  State required training will be provided.  Experience with Windows and Excel preferred.

ADDITIONAL REQUIREMENTS: Must submit official college transcripts.  Must have NC Driver’s license and access to a vehicle.
